The macrolides, derived from Streptomyces bacteria, are a group of antibiotics
that inhibits protein synthesis by bacteria at the 50S ribosome. They are
characterized by a macrocyclic ring (a large ring molecule with many functional
groups bonded to it) and a large lactone ring in the molecular structure. They
are usually used for respiratory tract, skin, and genitourinary infections.
Roxithromycin might be useful in treating toxoplasmosis, cryptosporidiosis,
isospora (an infection which, like cryptosporidiosis, causes severe diarrhea),
and possibly MAI. Cryptosporidiosis, a parasitic infection, often leads to
severe diarrhea and weight loss and is potentially fatal. Unfortunately, there
is no standard treatment for cryptosporidiosis. Roxithromycin is widely used in
Latin America and Europe as a safe and effective treatment for a number of
bacterial infections. Roxithromycin is similar to erythromycin in chemical
structure and mechanism of action. This preparations are made from microcoated
roxithromycin powder. This preparations, which are removed of bitter taste of
roxithromycin and absorbed well into the gastrointestinal tract, descrease the
side effects, but increase the safety.

protein synthesis. Roxithromycin binds to the subunit 50S of the bacterial
ribosome, and thus inhibits the translocation of peptides. Roxithromycin has
similar antimicrobial spectrum as erythromycin, but is more effective against
certain gram-negative bacteria, particularly Legionella pneumophila.
